OCFS2 File System Resource Fails in HA2 Cluster

Environment
Situation
An OCFS2 file system is created as an HA2 cluster resource. However, the resource goes unmanaged and won't activate or mount the file system. The following errors were observed:

Filesystem[13751]: [13778]: ERROR: /sys/kernel/config/cluster/ocfs2/node/nts123: failed to link
A directory listing shows the following:
# ls -l /sys/kernel/config/cluster/ocfs2/node/
total 0
drwxr-xr-x 2 root root 0 Aug 8 13:58 NTS123
drwxr-xr-x 2 root root 0 Aug 8 13:58 NTS124
drwxr-xr-x 2 root root 0 Aug 8 13:58 NTS126
Resolution
1. Edit /etc/ocfs2/cluster.conf on all nodes
2. Change the nodes names from mixed or uppercase to all lowercase
For example, change from
name = NTS123
to
name = nts123
3. Reboot all nodes
