Environment
SUSE Linux Enterprise Server 12 Service Pack 1 (SLES 12 SP1)
Situation
Mounting /sysroot
[FAILED] Failed to mount /sysroot
in the grub commandline there is an entry like
root=UUID=
for the root device.
Resolution
root=UUID=
with
root=/dev/mapper/insert_here_the_SCSIID_and_if_necessary_partNO_of_root_device
within the grub shell and boot into the system. After booting into the system
adjust the entry for the root Filesystem in
/etc/fstab
accordingly and set in

and then activate the new bootloader settings with
grub2-mkconfig -o /boot/grub/grub.cfg
the system will boot properly again.
